Polifarmasi dan Potensi Interaksi Obat pada Pasien Geriatri dengan Diabetes Melitus Tipe 2 dan Hipertensi di Instalasi Rawat Inap Rumah Sakit Kilisuci Kota Kediri

Latar belakang: Pasien geriatri dengan diabetes melitus tipe 2 dan hipertensi berisiko mengalami polifarmasi dan potensi interaksi obat. Tujuan: Mengidentifikasi karakteristik pasien, kejadian polifarmasi, potensi interaksi obat, tingkat keparahan interaksi, serta hubungan polifarmasi dengan potensi interaksi obat. Metode: Penelitian observasional analitik retrospektif ini menggunakan data rekam medis pasien rawat inap Rumah Sakit Kilisuci Kota Kediri tahun 2025. Sampel dipilih secara purposive berdasarkan kriteria usia ≥60 tahun dan kelengkapan data terapi, sehingga diperoleh 34 pasien. Potensi interaksi obat dianalisis menggunakan Medscape Drug Interaction Checker, dikonfirmasi dengan Drugs.com, serta diperkuat melalui DailyMed dan literatur ilmiah. Hubungan polifarmasi dengan potensi interaksi obat diuji menggunakan Fisher’s exact test. Hasil: Polifarmasi ditemukan pada 82,4% pasien. Potensi interaksi obat ditemukan pada 52,9% pasien dengan total 32 pasangan interaksi. Interaksi kategori moderate mendominasi, sedangkan major ditemukan pada sebagian kecil kasus. Tidak terdapat hubungan bermakna antara polifarmasi dan potensi interaksi obat. Simpulan dan saran: Potensi interaksi obat masih sering ditemukan. Medication review oleh farmasis perlu diperkuat dengan mempertimbangkan jenis obat, kondisi klinis, fungsi ginjal, dan risiko individual pasien.
